By booking a session, you gree to the following:

I understand that anything I share during sessions will be kept completely confidential, unless my coach/therapist has reason to believe that I may harm myself, or another person, or where there is a legal duty of disclosure.

I understand that my coach/ therapist may discuss my treatment with her supervisor, without revealing any identifying details.

I understand that any data held about me by my coach/therapist, such as contact details, case notes and correspondence, will be done so in accordance with the Data Protection Act 2018 and the General Data Protection Regulations (GDPR). I understand that the lawful basis for processing this information is ‘legitimate business interests related to the service being provided’.

I understand the payment terms of my sessions and that I will be charged half of the full session fee if I do not give 48 hours’ notice before cancelling my appointment.

I understand that my coach/therapist is bound by the ethics of the ACCPH Code of Conduct, which can be found here: https://www.accph.org.uk/code/of/conduct
